Kit Registration
After filling out the consent form and the kit code provided in the envelope, please enter the consent code located at the bottom of the form.

How can I find the Consent Code?
There is a QR code inside the kit that directs you to the consent form. After scanning the QR code with your phone, fill out the consent form on the page you are directed to. Once the form is completed, the code will be located at the bottom. The Consent Code starts with ECG.
Your kit has been successfully registered. You can track your process on the “PROCESS TRACKING” page.
An invalid code has been entered. Please make sure that both your Kit Code and Consent Code are correct.
